Martial Arts Gym Liability Waiver
I, {name}, acknowledge that participation in martial arts training and activities at GPX Holdings DBA Havok Martial Arts involves inherent risks, including but not limited to physical injury, muscle strains, joint injuries, or other harm, which may result from the physical nature of the activities, sparring, or use of equipment.  By signing this waiver, I:
  1. Acknowledge Risks: Understand and accept the risks associated with martial arts training, including the potential for serious injury or death.
  2. Assume Responsibility: Agree to assume full responsibility for any injuries, damages, or losses I may sustain while participating in activities at [Gym Name].
  3. Release and Waiver: Release, waive, and discharge [Gym Name], its owners, instructors, staff, and affiliates from any and all liability, claims, or causes of action arising from my participation, whether caused by negligence or otherwise.
  4. Health Declaration: Confirm that I am in good health and have no medical conditions that would prevent safe participation. I agree to inform the gym of any changes to my health status.
  5. Rules and Conduct: Agree to follow all gym rules, instructor directions, and safety protocols to minimize risks.
